About Sue Ryder Palliative Care Hub South Oxfordshire

Sue Ryder Palliative Care Hub South Oxfordshire operates from Battle Barns in Crowmarsh, Wallingford. We support people aged 18 and over who are living with life-limiting conditions such as cancer, heart failure and lung disease.

Our team of Sue Ryder Nurses, doctors, physiotherapists, occupational therapists, family support and befrienders combine specialist medical care for managing pain and other symptoms, with emotional, practical and spiritual support. We provide a Hospice at Home service, day hospice, and a care coordination and advice line.

Hospice at Home

The Hospice at Home service provides care for patients in their own home in the last 12 weeks of life. The service delivers expert care and support to relieve physical symptoms and emotional distress, as well as advising the patient’s family how best to help care for their loved one. Assistance is provided with personal care and repositioning.

The Hospice at Home service is available seven days a week, 8am-8pm and also offers a night service, giving care through the night between the hours of 10pm and 7am.

Clinical Nurse Specialists

The Clinical Nurse Specialist team provide advanced symptom management assessment in the home, coordinate planning of care and communicate with other Community Services, such as GP and District Nurses. Many of the Clinical Nurse Specialists are non-medical prescribers, which means they can prescribe at the bedside to provide urgent medications.

Day hospice

Our day hospice programme supports people living with a life-limiting condition requiring palliative care in South Oxfordshire. Our fully individualised approach is delivered by a team of experts offering small group sessions to best meet the needs of our patients, help manage symptoms and improve wellbeing.

Coordination and advice line

You can call our telephone line for specialist advice, support and care coordination. We speak to patients, families and carers, and healthcare professionals.

You’ll receive expert care and advice from a team of specialist Sue Ryder Nurses and consultants who work with GPs, community nursing teams and other palliative care providers.

I found that when the Sue Ryder team came out they always made John more comfortable. When John was still able to speak he would say to them, I don’t know why you do this, and they would say, ‘even small things can make such a difference’. They made everything a bit easier.
