Referrals and enquiries

Referrals to our services can be made by placement teams, health professionals, commissioners, case managers or family and friends.

To make a referral you can contact your closest centre directly either by telephone, email, or by faxing the information.

The clinical team will then review the information and if appropriate, will arrange for a face to face assessment to enable the team to accurately evaluate the persons specific and individual needs.
