Every participant must fill in their own registration form, and the parent or guardian should also sign if the participant is under 18. Registration is not transferable to any other person or event. The minimum number of participants in a team is 6 and maximum is 10. Participants may register for one team only and may not transfer to another team.
Health and Fitness –We ask that all participants satisfy themselves they are in good health when commencing their training programme and at the time of participation in the event. If in doubt, you should consult your GP or hospital doctor about your ability to engage in training in preparation for the Dance-a-thon and to take part in the event. If you are not a dance professional or a regular dancer or do not take any form of regular exercise, then we advise that you follow the recommendations of your doctor on getting fit for the event. If you are already reasonably fit, we suggest you go for a brisk walk, short run or do some other form of exercise two or three times a week for several weeks in advance of the Dance-a-thon and gradually increase the duration of your chosen exercise in preparation for the event.
Safety – All participants take part at their own risk. Whilst the organisers of the event will take all reasonable steps to make the event safe for you and your supporters, you must make sure that you wear suitable clothing and footwear. If you have not taken already taken part in a similar event, you are advised to bring trainers or other comfortable shoes in addition to your ‘dancing’ shoes. All participants must adhere to the guidelines set out in the Sue Ryder Dance-a-thon safety booklet (distributed at registration) as well as the instructions of the event organisers.
Disabilities - If you have a disability and wish to enter please speak to the organisers about how you can be supported in this event before completing this form.
Liability – The Dance-a-thon is operated by Sue Ryder. Sue Ryder does not accept responsibility for any loss, injury or damage suffered by any participant personally or to their property as a result of taking part in the Dance-a-thon. Personal accident and public liability insurance cover is advised for any loss, damage or injury you may sustain yourself or to your personal property or that you cause to a third party or their property.
Disqualification - Sue Ryder reserves the right to disqualify any individual participant or team not adhering to these terms and conditions or to the guidelines set out in the safety booklet or to event organiser’s directions. Disqualification of an individual participant may lead to disqualification of the team.
Fluid intake - It is the responsibility of the individual to ensure that they consume sufficient fluids for the duration of the Dance-a-thon and maintain a suitable level of hydration. Neither alcohol or alcopops may be brought into the venue.
Age – We regret the event is not open to under 16s, unless they have pre-registered and are aged 11 or over.
Publicity – Participants agree to take part in event publicity which may include interviews, photography and/or filming for Sue Ryder internal communications, promotions and/or advertising in local and/or national media of any description. Sue Ryder pledges that it will not use photographs or film of children in any promotional activity.
Contact details – Participants are personally responsible for updating the organisers with new address and telephone details.
Refunds – Participants withdrawing from the event will not be entitled to a refund.
Late entries – No registration on the day will be permitted. The last date that your registration will be accepted is [ ].
Fundraising Event – The Dance-a-thon is an exclusive Sue Ryder fundraising campaign. All participants must pledge to raise sponsorship for Sue Ryder. Sponsorship monies may not be given to any other organisation and participants are warned that they may be committing a criminal offence through not sending in sponsorship monies promptly.
Data Protection - Sue Ryder will process your personal and sensitive data in accordance with the Data Protection Act 1998. This may include using your likeness, a photograph or image of you or your quotes or a sound recording of you. Sue Ryder would like to keep you informed about our work and any future campaigns or events. If you would prefer not to be contacted please tick the appropriate box on the application form.
