The Community Activity Programme is a social rehabilitation programme for people aged 18 and over with a neurological or other long term health condition. It was developed in Doncaster in 1999 by Sue Ryder in partnership with Doncaster NHS and Doncaster Metropolitan Borough Council.

The majority of clients are referred to the programme from the local Stroke and Neuro Outreach teams although referrals are accepted from other voluntary organisations and by self referral.  The two Community Activity Programme Coordinators visit people in their own home to discuss the type of community based activities they would like to do and then we assist them on their social rehabilitation pathway.

We also organise the following Groups; Walking, Fishing and 10 Pin Bowling as we have found that for some people the groups provide the confidence they need to move on to other inclusive and meaningful community based activities.
