We don't just care for people at our hospices and in our neurological care centres; we also provide a variety of services from our expert teams out in the community, enabling people to remain in the comfort of their own homes. Treating people with the same level of care as they would receive at one of our centres.
From growing and developing our hospice community services to give our patients more choice and control over their care at the end of their lives, to offering community cafes for people with dementia and other long-term conditions.
All of our community services can share expert advice, support loved ones, and if need be, share details of other services that could help. Including our day, residential or inpatient services. And some, including our community cafes and self management programmes, offer activities in a safe and supportive environment, as well as the chance to meet and make friends with others who are going through similar experiences.
