Co-ordinating care (PEPS)
When diagnosed with a life-limiting condition, people often have many services involved in looking after them. To help make this as easy as possible for the patient and their family, we provide a co-ordination service in Bedfordshire, through an initiative called Partnership for Excellence in Palliative Support (PEPS).
For people in their last year of life, PEPS co-ordinates care between 15 different organisations. It provides a single point of contact via a 24-hour telephone service, making things easier for the individual, their family and carers.
In the future we hope to be part of expanding this approach as it makes it easier for people to get the right support at the right time, and to be cared for in the way they wish at the end of their lives.
