service for families living within South East Oxfordshire.

About our care
We offer a range of support, including inpatient care,
a seven-day community nursing service and day services, which
help people stay at home and provides respite to carers.
Our expert care team includes doctors, nurses, care

homes, our focus is on quality of life – giving every patient the
care and support they need to spend the time they have left
in the way they choose.
Who provides the care?
The service is delivered by a dedicated team of registered
nurses and nursing assistants employed by Sue Ryder.
They provide assistance with personal care e.g. assisted
washing, toileting and monitoring of symptoms.
When is it available?
The service is available Monday to Friday between 9am and 5pm.
How do you access the service?
Referrals to the service must be made via GPs, district
nurses or other healthcare professionals.

In order for our staff to support you to the best of their
abilities, we hope you will help us by providing some or

- easy/clear access to your house
- allowing the nurse to use your telephone, if necessary,
to report that they have arrived safely.
